What is a Track?
In Valera Health, tracks are preset schedules of questionnaires and/or educational programs that are sent to patients automatically.
For example, your team may decide that all patients with a primary diagnosis of mild-moderate depression should receive the following schedule of items:
- PHQ-9 questionnaire once a month
- 7-week mindfulness program with new episodes delivered weekly
With care plans, you can have these items sent automatically to a group of patients according to a schedule you set in advance. Wave goodbye to sending questionnaires manually or worse, leaving voicemails!
Tracks can be accessed by clicking on the tab in the top bar of the dashboard. You can then choose global or individual from there.
